Canada has many federal tax deductions and credits. This form will help you identify the ones that you may be eligible for. Please review the list and select “Yes” if you think it applies to your situation. If you are not sure what the item means, select “Unsure”. For each item that you answer “Yes” or “Unsure”, please provide some explanation in the space provided. You do not need to explain your “No” answers.

To support your claim, you must also attach official receipts, tax slips or other documents that show evidence of your eligibility. We require proof for all tax deductions and credits before we can claim them.
